About The Position

Our Phoenix Skilled Office is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Skilled Nursing team in either pediatric and geriatric private duty roles. We believe our clients and their families deserve the highest quality care- delivered with compassion, excellence, and reliability. This role involves 1:1 Home Care, providing traditional shifts (4’s, 8’s, 10’s or 12’s) with hands-on care for only one client per day, building client relationships and watching their milestones. You will travel to one client’s home near you on a schedule you choose, follow up with, execute, and properly document doctors' orders, perform assessments and monitor clients' conditions, and document observations, interventions, and evaluations, feeling empowered to perform skills with autonomy in the home.

Requirements

• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• A current nursing license in good standing in the state of Arizona
• One year of experience working as a Licensed Practical Nurse
