This position may provide direct nursing care to residents and supervises the day-to-day nursing activities performed by licensed practical/vocational nurses and certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. The role promotes holistic care for residents, ensures compliance with policies, and participates in resident admissions, discharges, and transfers. The LPN assesses changes in resident status, notifies physicians and families, transcribes physician orders, prepares and administers medications, and performs wound treatments. They also schedule follow-up appointments, provide direct care skills, perform rounds, and collaborate with the interdisciplinary team. The position involves participation in management team meetings, potential assistance in evaluating staff performance, reporting concerns and grievances, and participating in end-of-life care. Ensuring adequate stock of medications and supplies, initiating and updating care plans, promoting teamwork, and participating in survey processes and QAPI activities are also key responsibilities. The LPN ensures infection control principles are followed and corrects deficiencies.
