About The Position

The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN) in accordance with the Missouri Nurse Practice Act, state home health regulations, and agency policies. The LPN assists in implementing the patient's plan of care, provides treatments and interventions, monitors patient status, and communicates patient progress and changes to the supervising RN. The LPN works collaboratively with the interdisciplinary team to promote positive patient outcomes and ensure the delivery of safe, high-quality home health services.

Requirements

  • Graduate of an accredited Licensed Practical Nursing (LPN) or Vocational Nursing program.
  • Current and active Licensed Practical Nurse (LPN) license in the State of Missouri in good standing.
  • Minimum one (1) year of nursing experience preferred
  • Knowledge of home health regulations and clinical practices
  • Strong assessment and observation skills
  • Effective verbal and written communication
  • Ability to work independently in a home environment
  • Strong time management and organizational skills
  • Ability to collaborate effectively with an interdisciplinary healthcare team
  • Valid driver’s license
  • Reliable transportation with current automobile insurance in accordance with agency policy
  • Ability to travel to patient homes within the agency’s service area
  • Current CPR certification
  • Ability to meet background screening and health requirements as required by Missouri regulations
  • Ability to lift and move up to 50 pounds
  • Ability to stand, walk, bend, and assist patients with mobility
  • Ability to work in various home environments

Nice To Haves

  • Home health, hospice, or medical-surgical experience preferred

Responsibilities

  • Provides skilled nursing care to patients in accordance with the Missouri Nurse Practice Act, physician orders, and the established plan of care.
  • Implements the plan of care developed by the Registered Nurse, including nursing treatments, medication administration, wound care, and other clinical interventions as ordered.
  • Observes, records, and reports patient condition, including changes in physical, mental, and emotional status, to the supervising RN and appropriate healthcare team members.
  • Documents patient care accurately and timely in the agency’s electronic medical record (EMR) system in accordance with agency policy, Medicare/Medicaid guidelines, and regulatory requirements.
  • Assists with patient assessments and participates in the evaluation of patient progress toward established goals.
  • Provides education and instruction to patients, families, and caregivers regarding diagnoses, treatments, medications, safety, and the home care plan.
  • Participates in the coordination of patient services by communicating identified needs for additional disciplines such as: Home Health Aide (HHA) Physical Therapy (PT) Occupational Therapy (OT) Speech Therapy (ST) Medical Social Work (MSW) Dietitian
  • Maintains infection control and safety standards while delivering care in the patient’s home environment.
  • Uses medical supplies and equipment safely and efficiently, ensuring proper care and maintenance.
  • Maintains patient confidentiality and complies with HIPAA regulations and agency privacy policies.
  • Participates in case conferences, staff meetings, and in-service education programs as required.
  • Maintains required clinical competencies and continuing education to support professional growth and regulatory compliance.
  • Performs other duties as assigned by the Registered Nurse, Clinical Supervisor, or Administrator to support quality patient care and agency operations.
