This job post is exclusively for the role of staffing for these healthcare companies. In this role, the LPN will work at a crisis home with individuals with intellectual disabilities. The primary function of the Licensed Practical Nurse - Crisis is performing nursing activities, supports, care and treatment for individuals with Intellectual and Developmental Disabilities (IDD) in the Crisis home setting in accordance with the policies the State of Georgia Nursing Licensing Bureau. The Licensed Practical Nurse will be expected to work closely with an interdisciplinary team (Behavior Specialist, Psychiatrist, Psychologist, etc.), related service providers, families and other agencies as appropriate.

Requirements

  • Current and unrestricted LPN license in GA
  • 2+ years’ nursing experience preferred
  • Current CPR (American Heart Association) Certification
  • Proof of Negative TB test within the last 12 months
  • Valid GA drivers license
  • Valid Auto insurance and access to a reliable vehicle
  • Ability to work independently and with directions
  • Exceptional customer service skills
  • Compassionate and friendly demeanor

Responsibilities

  • Serves as the shift lead for the assigned home(s)
  • Assumes responsibility for the management of the unit
  • Ensures the unit is clean and equipment is in good working condition
  • Coordinates nursing care for individuals
  • Contacts RN and doctor as needed to report, clarify or discuss issues or concerns related to individual care.
  • Completes entire shift as assigned.
  • Assures that proper hand-off of essential information and status of care is provided to the oncoming nurse, including getting report and counting and signing off on narcotics.
  • Provides skilled nursing care, complete health assessments and evaluations as required
  • Administers and supervise the administration of medication
  • Ensures that the daily activity schedule is followed and that there is structured activity (skills training and education) occurring in the home
  • Performs medication management functions
  • Promptly reports changes in the individual’s condition to RN and family.
  • Ensures the unit is clean and equipment is in good working condition
  • Reviews doctor’s orders
  • Supervises and direct the activities of direct care staff agency staff
  • Ensure that the client is in a safe and productive environment at all times
  • Problem-solves issues and concerns as they arise in the home to ensure that the individuals’ needs are being met according to best practice
  • Communicates with pharmacy to order medications for individuals as needed
  • Ensures that monitoring of medications for individuals receiving services and assist staff with medication issues.
  • Reviews medication administration reports for assigned homes and supervise individual with self-administered medications.
  • Participate in staff meetings of crisis home staff
  • Meet with the RN on a routine basis to discuss and resolve current issues
  • Assists with interviewing, hiring, orientation and training of lower level staff for crisis homes.
  • Assists with performance appraisals and disciplinary actions
  • Review direct care staff progress notes and discuss with the appropriate staff member any discrepancies notes on the record (i.e. lack of documentation, inappropriate restrictions or lack of appropriate consequences; situations in which the current behavior program or treatment approach were not followed appropriately; check each report to ensure that time, date, signatures are correct
  • Maintain highly organized, consistent, thorough and systematic record keeping practices
  • Review and audit direct care staff records to ensure compliance
  • Provide consultation regarding crisis intervention and critical incident supports
  • Complete incident reports within 24 hours of incident, as required
  • Monitor house funds, i.e. making funds available for recreational events, document individual spending of funds, collecting receipts and follow GCSS administrative finance procedures and policies
  • Follow the weekly/monthly menu with individual assistance.
  • Collaborate with RN, to ensure the homes are managed well and run efficiently and in accordance to all applicable Rules and Regulations
  • Participate in professional growth activities such as conferences, classes, team meetings and program visitations
  • This position directly supervises the activities of the Direct Support Professional (DSP) and Registered Behavioral Technicians (RBT) and agency staff.
  • Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws.
