About The Position

Brookside Village is hiring a full-time, night-shift Licensed Practical Nurse (LPN). The position provides high-quality care to residents, supports their comfort and well-being, and helps advance their health goals. The LPN collaborates with a multidisciplinary healthcare team to develop and implement individualized care plans, monitors resident health, administers medications, and educates residents and their families. A sign-on bonus of up to $10,000 is available. Requirements

Current and valid Licensed Practical Nurse (LPN) license in the state of Indiana.
Basic Life Support (BLS) certification.

Nice To Haves

Experience in a long-term care or assisted living environment.
Proficiency in electronic health record (EHR) systems.

Responsibilities

Administer medications and treatments as prescribed by the healthcare provider.
Monitor and record vital signs and report changes in residents’ conditions to nursing staff.
Assist residents with daily living activities, including bathing, dressing, and grooming.
Maintain accurate and up-to-date resident records and documentation.
Collaborate with healthcare team members to develop and implement care plans.
Provide education to residents and their families.
Support residents’ comfort, well-being, and health goals.
