About The Position
Elara Caring is hiring a Licensed Practical Nurse for Personal Care Services in the Jefferson City, Missouri branch, serving Jefferson City, Columbia, and surrounding areas. The role provides nursing care in patients’ homes, supporting the company’s mission to deliver care in the patient’s preferred home setting. Travel is required for approximately 50% of the role. The position offers a $2,500 sign-on bonus. The nurse delivers care in alignment with patient care plans, reviews the Plan of Care before visits, assists with medication management and documentation, and communicates patient updates and changes in condition. The role also includes educating patients and families, identifying safety concerns, and collaborating with RN Case Managers, physicians, and the broader care team. The position reports to the Clinical Manager or Branch Director for Personal Care Services.
Requirements
- Current LPN/LVN license in the state of Missouri.
- Graduation from an accredited nursing program.
- At least 1 year of experience in acute care.
- Strong communication and interpersonal skills.
- Ability to manage time effectively and stay organized in the field.
- Knowledge of healthcare regulations and patient care standards.
- Compassionate, patient-first mindset.
- Reliable personal transportation; travel required 50%.
- Ability to meet health screening and compliance requirements.
Nice To Haves
- Experience in home care, home health, or hospice is preferred.
Responsibilities
- Deliver in-home nursing care aligned with patient care plans.
