About The Position

Elara Caring is hiring a Licensed Practical Nurse (LPN) for Personal Care Services in Jefferson City, Missouri, serving Jefferson City and surrounding areas. The role provides in-home nursing care in accordance with patient care plans and supports patients in their preferred home setting. The position offers a $2,500 sign-on bonus.

Requirements

• Current LPN/LVN license in the state of Missouri.
• Graduate of an accredited nursing program.
• At least 1 year of experience in acute care; home care, home health, or hospice experience is preferred.
• Strong communication and interpersonal skills.
• Ability to manage time effectively and stay organized in the field.
• Knowledge of healthcare regulations and patient care standards.
• Compassionate, patient-first mindset.
• Reliable personal transportation; travel is required 50%.
• Ability to meet health screening and compliance requirements.

Responsibilities

• Deliver in-home nursing care aligned with patient care plans.
• Review and follow the Plan of Care before each visit.
• Communicate patient updates, concerns, and changes in condition.
• Assist with medication management and documentation.
• Educate patients and families on self-care techniques.
• Identify safety concerns and help prevent hospitalizations.
• Collaborate with RN Case Managers, physicians, and the care team.
