About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The role is part of a team of associates who support one another and build relationships with residents and colleagues. The position is per diem. The LPN assesses and documents residents’ status, communicates significant changes, supervises and mentors Resident Care Assistants, coordinates care with community providers, and responds to emergencies. The nurse balances service needs with supporting resident independence and helps prevent and report elder abuse, neglect, and exploitation.

Requirements

Current state license as an LPN.
CPR certification.
Graduate of an approved LPN program, per state requirements.
One to two years of nursing experience is preferred.
Previous experience working with older adults in a paid or volunteer position is highly desirable.

Responsibilities

Accurately assess, communicate, and document residents’ status.
Observe and report significant changes in residents’ behavior and health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, in accordance with state regulations.
Direct and supervise the daily work assignments of Resident Care Assistants.
Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
Coordinate care needs with community providers through an effective case management process.
Balance service demands with support for resident independence.
Recognize and assist in preventing elder abuse, neglect, and exploitation, and report concerns to appropriate sources in accordance with state regulations.
Respond to emergencies and personal emergency response systems, and know when to call for backup.
