About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care to residents in a warm, comfortable, home-like environment. The LPN works with a supportive team and helps build lasting relationships with associates and residents. The position also involves supervising and mentoring Resident Care Assistants, coordinating resident care, supporting resident independence, and responding to emergencies.

Requirements

• Current state LPN license.
• CPR certification.
• Graduate of an approved LPN program, as required by the state.

Nice To Haves

• One to two years of experience working as a nurse is preferred.
• Previous experience working with elderly people in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assess, communicate, and document residents’ status.
• Observe and report significant changes in resident behavior or health to the Resident Care Director, physician, family, Resident Care Assistants, and Executive Director, in accordance with state regulations.
• Direct and supervise the daily work assignments of Resident Care Assistants.
• Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
• Coordinate care needs with community providers through an effective case management process.
• Balance service demands while supporting resident independence.
• Recognize and help prevent elder abuse, neglect, and exploitation, and report concerns to appropriate sources per state regulations.
• Respond to emergencies and personal emergency response system alerts, and determine when to call for backup.
