About The Position

The Licensed Practical Nurse (LPN) performs a variety of professional nursing duties in compliance with the Skilled Nursing Facility regulations and follows established policies and procedures for our facility in Burnsville, MN- Ebenezer Ridges Care Center. The nurse weekend schedule includes 32 hours per two weeks, with day/evening shifts. This role requires working every other weekend (Sat/Sun) and holiday rotation. Ebenezer Ridges Care Center offers a full continuum of care campus including Transitional Care, Adult Day Programs, Intergenerational Child Care, Long Term Care, Enhanced Care and Assisted Living/Memory Care. We are located across the street from Fairview Ridges Hospital at the I-35E and I-35W split in Burnsville, MN. Requirements

MN LPN licensure
AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers (3 week grace period for BLS)
Able to work independently
Good communication and interpersonal skills

Nice To Haves

Previous healthcare experience
Comfortable using computers, hand held devices, phones and fax machine

Responsibilities

Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practices
Lifts and transfer residents as needed, Initiates emergency treatment as needed
Monitors work and performance of unlicensed staff
Updates RN with current observation and completes documentation of resident health status
Communicates changes in residents conditions to relevant individuals
May support RN by completing resident assessments, service agreements and care plans
