The Licensed Practical Nurse (LPN) will be responsible for completing all documentation related to client assessments, observations, and care plan evaluations. This role involves re-evaluating clients regularly to ensure their needs align with the established care plan. The LPN will use professional assessment skills to identify clients' physical, mental, and psychosocial needs and update care plans accordingly. Additionally, the LPN will oversee caregivers, ensure adherence to AHCA standards, and serve as a mentor to peers and subordinates to uphold professional LPN standards. This position requires acting on behalf of clients to protect their rights and participating in an on-call rotation, including weekends. Responsibilities include calling caregivers during on-call duty, documenting caregiver reports, and ensuring home safety awareness among staff. The LPN will promptly respond to client complaints regarding caregivers, document these complaints, discuss them with the scheduling team, and report to the Nurse and office. Daily/weekly agendas must be documented on the office shared calendar in advance and updated as needed. Collaboration with the Director of Nursing (D.O.N.) is required for monthly and/or quarterly mandated reports. Weekly visits to clients, caregivers, and floaters in Preferred Provider buildings are necessary, with updates documented on computers and in a weekly nursing summary. Visits to clients in hospitals or rehabilitation centers are also part of the role, involving communication with nursing staff for updates. For medication management clients, visits every two weeks are required to fill pill planners or medication dispensers and call in necessary refills. Medication verification must be submitted to the doctor every 60 days for signature. Mandatory staff meetings are held every Monday at 10 am. Timely responses to emails and texts, especially while on call, are crucial. The LPN will report to their direct supervisor for task clarifications and will complete annual evaluations and 3-year competencies for caregivers, ensuring they are signed off and returned to HR.
