Licensed Practical Nurse (LPN) - Primary Care
About The Position
Within the Nursing Professional Practice Framework, the Licensed Practical Nurse provides quality, cost-effective direct care for patients and families within a unit, service, or other primary area of practice. The LPN collaborates with the healthcare team to help patients achieve established care outcomes and supports individual and family health goals through patient data collection, documentation, reporting, and therapeutic measures performed under the direction of an RN or MD. Requirements
- Current Connecticut Licensed Practical Nurse license.
Nice To Haves
- Medical/surgical office experience preferred.
- Pediatric experience preferred.
Responsibilities
- Collaborate with members of the healthcare team to assist patients in achieving established outcomes of care.
- Participate in direct patient care to meet individual and family health goals by obtaining, documenting, and reporting patient data related to the established plan of care.
- Perform therapeutic measures under the direction of an RN or MD to assist in achieving established patient care outcomes.
