About The Position
Hartford HealthCare Medical Group is seeking a Licensed Practical Nurse (LPN) to provide quality and cost-effective direct care for patients and families within a primary care setting. The LPN will collaborate with the healthcare team to assist patients in achieving established outcomes of care, participate in direct patient care to meet health goals by obtaining, documenting, and reporting patient data, and perform therapeutic measures under the direction of an RN or MD.
Requirements
- Current Connecticut Licensed Practical Nurse license required.
Nice To Haves
- Medical/Surgical office experience preferred.
Responsibilities

- Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
