About The Position

Riverside Community Care is seeking a Licensed Practical Nurse (LPN) for their Community Crisis Stabilization (CCS) program within the Behavioral Health Services Division in Upton, MA. This program serves up to 6 individuals at a time. The role involves working as part of a cohesive, experienced, multi-disciplined team in a voluntary community crisis stabilization program and acute care residence. Major responsibilities include assisting the nurse manager with physician orders, administering medications, and monitoring vital signs. The LPN will provide guidance, education, supervision, and direct support to enable consumers to maintain their physical and emotional health, live safely in the community, and develop skills for independent living. Occasional home visits to assess needs are also required.

Requirements

• LPN required
• Minimum of one year experience in the mental health field required
• Excellent communication, organizational, and time-management skills required
• Basic computer fluency (Microsoft Office) required
• Driver's license required
• Must be able to lift ten pounds

Responsibilities

• Assisting the nurse manager with filling physician orders
• Administering medications
• Monitoring vital signs
• Providing guidance, education, supervision, and direct supports to consumers
• Performing occasional home visits to assess needs
