About The Position

This role involves providing direct patient care as a Licensed Practical Nurse (LPN) on the 2nd or 3rd shift. Responsibilities include administering medications, performing treatments, documenting resident conditions, communicating with physicians and families, and ensuring the comfort and quality of life for residents. The LPN will also oversee and support Certified Nursing Assistants (CNAs) and collaborate with other departments and therapy services.

Requirements

  • Graduate of an accredited school of nursing.
  • Licensed by South Carolina Board of Nursing by examination or reciprocity.

Responsibilities

  • Make daily assignments if in charge of the shift.
  • Receive report from the outgoing nurse and conduct walking rounds.
  • Administer medications as scheduled.
  • Perform scheduled treatments.
  • Complete daily and PRN documentation.
  • Complete incident reports and behavior monitor logs.
  • Notify MD for any changes in residents' status or abnormal labs.
  • Receive and transcribe physician's orders.
  • Request lab work, schedule X-rays, and doctor's appointments.
  • Notify responsible party for incidents, changes in condition, or scheduled appointments.
  • Make transportation arrangements for residents.
  • Review labs and place them in the chart (7 AM-3 PM shift).
  • Document weights and report significant changes to dietary (3 PM-11 PM shift).
  • Receive, check, and sign deliveries from the pharmacy (3 PM-11 PM shift).
  • Chart vital signs (11 PM-7 AM shift).
  • Check and prepare MARs and treatment books for the next month (11 PM-7 AM shift).
  • Fax orders to the pharmacy on Saturdays, Tuesdays, and Thursdays.
  • Check emergency carts and expiration dates on multidose vials daily.
  • Defrost and clean refrigerators and med carts (Sunday 11 PM-7 AM shift).
  • Make copies of the daily worksheet and midnight census and distribute them.
  • Ensure CNAs perform their assigned tasks according to the facility's standard of care.
  • Discipline CNAs according to the Employees' Code of Conduct.
  • Handle call-ins and fill in the sheet.
  • Assist in admitting and discharging residents.
  • Start IVs and give IV antibiotics if certified (but not mix them or do IV push).
  • Ensure dietary supplements and bedtime snacks are given to residents in a timely manner.
  • Perform all aspects of bedside care for resident comfort and quality of life.
  • Assist with call lights.
  • Develop harmonious working relationships with other departments.
  • Communicate with DON, ADON, supervisors, and MDS Coordinator regarding residents' care.
  • Work well with physical, occupational, and speech therapies.
  • Perform other routine and special functions as assigned or necessary.
  • Follow chain of command.
