About The Position

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. Requirements

  • Is a graduate of an accredited practical nurse or vocational nursing program.
  • Has at least two (2) years of nursing experience.
  • Is currently licensed as an LPN through the State Board of Nursing.
  • Has excellent assessment, nursing judgment, and communication skills.
  • Able to cope with heightened family emotional stress, and tolerant of individual lifestyles.
  • Complies with accepted professional standards and principles.
  • Complies with strict adherence to organizational policies, procedures, and protocols.
  • Basic computer skills are required.
  • Is self-directed with the ability to work with little supervision.
  • Is flexible and cooperative in fulfilling role obligation and has the ability to work with an interdisciplinary team.
  • Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Nice To Haves

  • Community Health/Hospice or long-term care experience preferred.
  • Telehealth experience also preferred.
  • Advanced computer skills are preferred.

Responsibilities

  • Performs telehealth triage functions including but not limited to receiving calls; providing phone support; providing interventions utilizing BCN policies, procedures, and protocols; dispatching on-call nurses as needed.
  • Completes documentation utilizing the EMR, point of service.
