About The Position

This role is for a Licensed Practical Nurse (LPN/LVN) or Registered Nurse (RN) to join the future of e-health in a Remote Patient Monitoring (RPM) position. The nurse will manage a panel of patients with chronic conditions, reviewing their daily vitals, identifying trends, conducting monthly check-in calls, and escalating concerns to providers. This position offers a work-from-home environment with a predictable Monday to Friday schedule, no nights, weekends, or holidays, and a steady pay rate. It provides an opportunity to use nursing skills differently by building relationships with patients over time and offers potential for career growth within the company.

Requirements

Active, unrestricted LPN/LVN or RN license.
2+ years of nursing experience.
Compact Nursing License required.
Strong clinical assessment skills with chronic disease populations (HTN, DM, CHF, COPD).
Excellent phone communication skills; ability to build rapport quickly and explain complex information simply.
Comfort with technology, including EHRs, RPM platforms, and video conferencing.
Self-directed and able to manage a patient panel independently.

Responsibilities

Review daily vitals (BP, weight, glucose, SpO2) from the patient panel and act on findings.
Triage out-of-range readings and escalate to the supervising provider with a clear recommendation.
Document every review, intervention, and call to meet CMS billing requirements.
Conduct monthly check-in calls to assess symptoms, review medications, reinforce the care plan, and answer questions.
Onboard new patients and guide them through using their devices.
Act as a liaison between the patient and the provider's office, relaying concerns, following up on orders, and closing the loop.
Identify access barriers (transportation, cost, caregiver support) and connect patients to resources.
