About The Position
We're looking for a Home Health LPN to provide practical nursing services under the supervision of a Registered Nurse.
Requirements
- Holds a current Licensed Practical Nurse license in Virginia
- Has a minimum of one year of nursing experience
- Has current CPR certification
- Has excellent communication, organizational, and human relations skills
- Is able to function with minimal supervision
- Is able to manage multiple projects/changing priorities
- Is able to employ critical thinking skills
Responsibilities
- Accepts assignments of clients from the R.N. Case Manager
- Provides nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans
- Performs treatment(s) ordered by the physician and delegated by the R.N. Case Manager.
- Observes signs and symptoms and reports them to the R.N. Case Manager.
- Notes changes in physical or emotional status of clients.
- Promotes client and caregiver independence in self-care
- Utilizes electronic documentation of client care; complies with Agency policies regarding timeliness in submission of documentation and synchronization of data
- Follows Agency policies regarding universal precautions, and safety guidelines
- Fulfills other assignments as requested by the R.N. Case Manager, VP of Clinical Services, or Director of Professional Services.
