The LPN/LVN administers nursing care according to the plan of care to terminally ill patients admitted to the VITAS program, in accordance with the Nurse Practice Act in the State where the VITAS program is located. This role involves providing direct skilled bedside nursing care, evaluating patient/family needs, and assisting in the preparation and execution of the interdisciplinary plan of care. The LPN/LVN performs selected acts in the care of the terminally ill under the supervision of a registered nurse, including administering treatments and medications, evaluating their effectiveness, and reporting significant findings or changes in patient condition. Accurate documentation in clinical notes and on interdisciplinary charts is essential, as is maintaining a clean and safe environment. The position requires establishing therapeutic relationships with patients/families, involving them in the plan of care, and initiating nursing protocols for physical comfort and mental well-being. Participation in patient/family teaching, professional growth activities, and on-call rotations are also key aspects of the role.
