About The Position
Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) in the Ft Myers, FL area. This role offers the opportunity to take control of your schedule while providing meaningful one-on-one care in patients' homes. The coverage area is Ft Myers, FL, with territories designed to minimize drive time through clustered scheduling. The position utilizes a pay-per-visit model with consistent patient volume provided. LPNs choose Concierge Home Care for its flexibility, autonomy, and work-life balance, avoiding hospital shifts or 12-hour rotations. Requirements
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license
- Valid driver’s license, reliable transportation, and auto insurance
- CPR certification

- Home health experience preferred, but not required
Responsibilities
- Provide skilled nursing care under the supervision of a Registered Nurse
- Perform patient assessments and assist with care planning
- Administer medications and treatments as ordered
- Educate patients and caregivers on disease management and care plans
- Monitor patient condition and report changes to the supervising RN
- Collaborate with physicians and interdisciplinary teams
- Complete documentation using streamlined, AI-assisted tools
