About The Position

Hospice of the Western Reserve, now Reserve Care, is seeking a Licensed Practical Nurse (LPN) for weekend on-call duties. This role offers the opportunity to make a significant difference in the lives of patients and families during critical times. As an On-Call Nurse, you will have the autonomy to utilize your nursing skills, providing care and support to patients and families who need assistance after hours. You will be invited into patients' homes and lives during uncertain times, becoming a trusted face and providing compassionate care. Requirements

  • High School Diploma or equivalent required.
  • Graduate of an accredited school of nursing.
  • Licensed RN in state of Ohio.
  • Ability to communicate effectively, orally and in writing.
  • Ability to work independently and collaboratively with a team.
  • Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
  • Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere.
  • Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow filled walkways, etc.).
  • Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology.
  • Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR’s insurer.
  • Ability to stand and walk for extensive periods of time.
  • Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment.
  • This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics when working with patients.
  • Ability to stoop, squat, or bend frequently.
  • Corrected vision and hearing within normal range to observe and communicate with patients and professional staff.
  • Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation.
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed.
  • Provide an active driver’s license.
  • Ability to provide proof of eligibility to work in the United States.
  • High level of integrity, ethics and professionalism.

Nice To Haves

  • Bachelor's degree preferred.
  • Experience preferred in pain and symptom management, oncology, or mental health.

Responsibilities

  • Provide intermittent skilled nursing services to the patient under the patient’s physician’s direction.
  • Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
  • Serve as a liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs.
  • Provide emotional support to the patient and family.
  • Refer patients and/or family members to other services when appropriate.
  • Provide instruction (health care teaching) to the patient and family as related to care in a residence.
  • Make visit to patient’s residence at the time of death.
  • Maintain, promptly complete, and return all documentation as required.
  • Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
  • Relay pertinent information to appropriate team members.
  • Participate in meetings as initiated by team leader.
  • Participates in mandatory or elective RC University events, trainings, or in-services as necessary.
  • Perform other duties as assigned.
