Licensed Practical Nurse (LPN) - No Weekends
About The Position
We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in New Castle, IN. This is a case management role with semi-annual physical assessments. The environment is described as low stress, relaxed, with high morale, family-like, and team-oriented. Dungarvin assists people with intellectual disabilities, developmental disabilities, physical disabilities, autism and/or mental health diagnosis in a variety of programs with a focus on person-centered practices. Requirements
- Current Licensed Practical Nurse (LPN) license in the State of Indiana required
- Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
- Strong verbal/written communication skills
- Ability to work independently and efficiently
- Organizational and computer skills
- Acceptable background check & driving record check
Nice To Haves
- 1-2 years experience working with individuals with disabilities or other long-term care is preferred
Responsibilities
- Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
- Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress
- Act as a health/medical resource for staff, training staff in the medical arena
- Conduct audits and reviews of medications
- Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records
- Participate in an on-call rotation for triage of medical concerns for 1 week per month
