About The Position

We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities. This role involves ensuring each individual receives quality, individualized health care management, conducting routine site visits, checking vitals, doing assessments, counseling on medication administration, and documenting progress. The LPN will also act as a health/medical resource for staff, training them in the medical arena, conducting audits and reviews of medications, and ensuring compliance with state requirements and organizational policies regarding medical care and records. The position offers flexible hours, a 5-day work week (40 hours, Monday through Friday), and a low-stress, relaxed environment. It is a case management role with semi-annual physical assessments, a high morale, family-like, team-oriented environment, and paid training.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of Indiana required
  • 1-2 years experience working with individuals with disabilities or other long-term care is preferred
  • Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
  • Strong verbal/written communication skills
  • Ability to work independently and efficiently
  • Organizational and computer skills
  • Acceptable background check & driving record check

Responsibilities

  • Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
  • Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress
  • Act as a health/medical resource for staff, training staff in the medical arena
  • Conduct audits and reviews of medications
  • Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records
  • Participate in an on-call rotation for triage of medical concerns for 1 week per month
  • Only on call for triage of emergencies 1 week per month
