About The Position

StoryPoint Crystal Lake is seeking a part-time Licensed Practical Nurse (LPN) for the 3rd shift (10P-6:30A). This role involves leading, managing, and training a team of caregivers, evaluating residents, reporting changes in condition to physicians, utilizing Electronic Health Records (EHR) and Electronic Medication Administration Records (EMAR), communicating effectively with the team, residents, and families, and building strong relationships. Requirements

• Licensed Practical Nurse (LPN) in the state.
• Prior supervisory experience required.
• Punctuality and professionalism are important factors.
• Passionate about working with the elderly.

Nice To Haves

• Assisted Living/Memory Care experience preferred.
