Licensed Practical Nurse (LPN)
About The Position
Interim HealthCare is seeking a dedicated Licensed Practical Nurse (LPN) to provide one-on-one nursing care for an adult female client in her home in Plaistow, New Hampshire. This position offers the unique opportunity to develop a meaningful relationship with your patient while delivering skilled nursing care in a comfortable home setting. This role is ideal for LPNs who enjoy working independently, value continuity of care, and take pride in making a lasting difference in someone's quality of life. The client requires skilled nursing support and ongoing monitoring related to epilepsy and seizure management, history of transient ischemic attacks (TIAs), cerebral palsy, medication administration and symptom monitoring, and ongoing health assessment and support.
Requirements
- Active Licensed Practical Nurse (LPN) license in the State of New Hampshire
- Current CPR certification required
- Strong clinical assessment and communication skills
- Ability to work independently and confidently in a home setting
Nice To Haves
- Home care experience preferred
- Experience caring for patients with neurological conditions is a plus
Responsibilities
- Provide skilled nursing care in accordance with the established care plan
- Administer medications and treatments as ordered
- Monitor patient condition and promptly identify changes in health status
- Respond appropriately to medical concerns and emergencies
- Collaborate with physicians, nurse practitioners, case managers, and other members of the healthcare team
- Maintain accurate and timely clinical documentation
- Provide compassionate support that promotes dignity, comfort, and independence
