About The Position

Affirmed Home Care is seeking dedicated Licensed Practical Nurses (LPNs) to provide personalized, one-on-one care in patients' homes. The agency is looking for experienced LPNs with backgrounds in Med-Surg, Long-Term Care, or Home Care. This role offers flexible per diem shifts and patient visit assignments, emphasizing a supportive environment where nurses' skills are valued and their impact is significant. Requirements

• Minimum of 1 year recent LPN experience (acute care, long-term care, or home care)
• Active New Jersey LPN license
• Current BLS certification
• Authorization to work in the U.S.
• Physical exam within the past year
• PPD or QuantiFERON within the past year (or chest X-ray within 5 years)
• MMR immunization within the past 10 years
• Proof of flu vaccination or approved medical exemption
