Human Touch Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing team serving patients throughout Richmond, Henrico, Chesterfield, Mechanicsville, Glen Allen, Midlothian, Hanover, and surrounding communities. As a Home Health Licensed Practical Nurse, you'll provide skilled nursing care in the comfort of your patients' homes while helping them manage chronic conditions, recover from illness, and remain safely where they want to be. Requirements

Active Virginia Licensed Practical Nurse (LPN) License Required
Valid Driver's License and Reliable Transportation
Strong Clinical, Communication, and Documentation Skills
Compassionate, dependable, and committed to delivering exceptional patient care

Nice To Haves

Home Health Experience Preferred but Not Required

Responsibilities

Provide skilled nursing care under the direction of a Registered Nurse and physician
Administer medications and perform treatments as ordered
Monitor patient conditions and report changes to the supervising RN
Assist with disease management and patient education
Complete wound care and other skilled nursing procedures within your scope of practice
Collaborate with physicians and the interdisciplinary care team
Complete timely and accurate clinical documentation
