About The Position

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our team in a long-term care environment. The ideal candidate will have strong clinical skills and a commitment to providing high-quality care to residents. This is a newly opened facility, established in November 2016, and serves a population of 1600 residents. The working environment is described as slow-paced, allowing for effective resident care.

Requirements

• Current and valid LPN license.
• Experience in long-term care (LTC) settings is mandatory.
• Ability to work independently and as part of a multidisciplinary team.
• Strong communication and interpersonal skills.
• No facial piercings allowed during shifts.
• Tobacco-free facility: smoking is not permitted during shifts.

Responsibilities

• Administer medications and treatment as prescribed and ensure accurate documentation using Point Click Care Comp charting.
• Monitor resident health status and report any changes to the nursing team.
• Provide compassionate care and support to residents and their families.
• Assist with daily living activities and personalize care plans based on individual resident needs.
• Collaborate with a nursing team and coordinate care with CNAs.
• Maintain a clean and safe environment, adhering to all health and safety regulations.
