Requirements
- Current Licensed Practical Nurse (LPN) license in the state of employment
- Minimum of one year of LPN experience in an acute care setting
- Valid driver’s license and active auto insurance
- Ability to meet physical job requirements
Nice To Haves
- Home health experience preferred
Responsibilities
- Deliver skilled nursing care according to physician orders and care plans
- Monitor patient conditions and communicate changes appropriately
- Administer medications and treatments as prescribed
- Educate patients and families on disease processes and preventative care
- Complete accurate and timely clinical documentation
- Follow agency policies, procedures, and Medicare/Federal guidelines
- Participate in trainings and team meetings
