About The Position

Human Touch Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing home health team serving Allentown and surrounding Lehigh Valley communities, including Whitehall, Bethlehem, Emmaus, Macungie, Catasauqua, Coplay, Fountain Hill, and nearby areas. This PRN nursing opportunity offers flexibility while allowing you to build meaningful relationships with your patients. In home health, you're focused on one patient at a time, providing skilled nursing care in patients' homes while helping them manage their health, recover safely, and maintain as much independence as possible. You'll have the opportunity to truly get to know your patients.

Requirements

Active Pennsylvania Licensed Practical Nurse (LPN) License Required
Current CPR/BLS Certification
Valid Driver's License and Reliable Transportation
Strong Nursing, Communication, and Documentation Skills
Ability to Work Independently in Patients' Homes

Nice To Haves

Home Health Experience Preferred, but Not Required

Responsibilities

Provide skilled nursing visits in patients' homes
Perform focused nursing assessments and report changes in patient condition
Administer medications and treatments as ordered and within the LPN scope of practice
Provide wound care and monitor healing
Reinforce disease-management and medication education
Monitor vital signs and patient response to treatment
Educate patients and caregivers on safety and the established plan of care
Communicate changes and concerns promptly to the supervising RN and care team
Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary team
Complete timely and accurate clinical documentation using WellSky
