About The Position

Requirements
- Active Tennessee LPN License.
- Valid driver's license.
- BLS Certification within 30 days of hire (training available).
- PCM Certification within 60 days of hire (training available).
- Ability to lift and assist with mobility needs up to 50 lbs.
- Ability to pass a background check and drug screening.
Responsibilities
- Administer medications and treatments.
- Monitor health conditions, vital signs, and documentation.
- Respond to medical concerns and emergencies.
- Support physician appointments and follow physician orders.
- Train and support staff on health-related needs.
- Participate in Individual Support Plans (ISPs) and interdisciplinary team meetings.
- Assist with daily living needs when necessary.
- Help ensure the safety, health, and well-being of the individuals we serve.
