About The Position

Aurora Valley Care Skilled Nursing & Rehab, a Caldera Care facility, is seeking Licensed Practical Nurses to provide nursing care to residents in a skilled nursing and rehabilitation setting. Full-time and PRN positions are available, with evening and NOC shifts. The listed pay range is $38–$43, depending on experience. The LPN provides and documents resident care, administers ordered medications and treatments, completes nursing assessments and progress documentation, and communicates changes in residents’ conditions to providers and nurse management. The role also supports residents and families with health care instructions, collaborates with the care team, assists with admissions, transfers, and discharges, and directs unit staff as needed. The position reports to the Unit Nurse Manager.

Requirements

Current and valid LPN license.
Multistate license holders must have the 6-Hour Suicide Prevention Certificate.
Excellent interpersonal skills.
Fast and adaptive problem-solving abilities.
In-depth understanding of patient care policies and regulations.
First aid and CPR certifications.

Responsibilities

Provide nursing care to patients on the assigned unit.
Administer and record medications and treatments as ordered.
Complete nursing assessments, evaluations, progress notes, and alert charting as necessary.
Educate residents and families and provide health care instructions as necessary.
Collaborate with team members to plan, implement, and evaluate nursing care and other patient activities.
Assist with patient admissions, transfers, and discharges as necessary.
Communicate with providers and nurse management about patients’ conditions, changes, medications, and vital signs.
Provide direct care to residents as needed.
Direct unit staff as necessary.
Report resident and staff issues or concerns to the direct supervisor, the Unit Nurse Manager, as needed.
