About The Position

St. Joseph Hospice is hiring for Full-Time Licensed Practical Nurses (LPN) in Lake Charles, LA! Reporting changes in patient condition to the RN is crucial, and great time management skills are essential.

Requirements

Current Louisiana Licensed Practical Nurse (LPN) license required
Minimum of 2-3 years as a Licensed Practical Nurse (LPN)
Current CPR certification required
Have dependable transportation, driver’s license, and car insurance

Nice To Haves

Previous Hospice experience preferred

Responsibilities

Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
Document all services provided and submit them on appropriate forms.
Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
Report changes in patient condition and needs to RN.
