About The Position

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in a skilled care setting in Phoenix, AZ. This is a full-time opportunity offering competitive pay and consistent scheduling.
Schedule:
Night Shift: 6:30 PM – 6:00 AM (40 hours per week)
Day Shift: 6:00 AM – 6:30 PM (30 hours per week)
12-hour shifts

Requirements

• Active LPN license (State of Arizona required)
• Knowledge of geriatric/long-term care practices preferred
• Strong communication and interpersonal skills
• Ability to assess, respond, and make sound clinical decisions
• Ability to work effectively in a team environment

Responsibilities

• Administer medications and perform treatments as ordered by the physician and within scope of license
• Monitor and report effectiveness of medications and treatments
• Maintain adequate supplies and medications
• Communicate resident needs with interdisciplinary team, including family and staff
• Complete admission assessments and assist with care planning (MDS)
• Maintain accurate clinical and administrative documentation
• Participate in shift reports and team collaboration
