About The Position

Works under the supervision of a Registered Nurse in the care of patients and their environment, including those needing special precautions, and performs specified treatments. This position requires the ability to provide age-appropriate care based on the growth and development principles of the patient population served. The LPN promotes a safe work environment and incorporates national patient safety initiatives. They may work in various settings within Riverside Health System, collaborating with leadership and a multi-disciplinary team to provide comprehensive clinical care in accordance with Riverside Health System standards and facility goals.

Requirements

  • Program Graduate, Practical Nursing (Required)
  • High School Diploma or GED (Required)
  • Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required)

Responsibilities

  • Under the supervision of the RN, partners with patients and families to implement the plan of care, treatments, and procedures that contribute to the physiological and psychosocial outcomes of the patient.
  • Incorporates evidence-based standards of practice.
  • Collaborates with the interdisciplinary team in the coordination of care.
  • Facilitates safe patient transitions to the next care setting as part of the plan of care.
  • Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care.
  • Focuses on quality outcomes of patient care when addressing customer’s needs, including current evidence-based practice and quality measures.
  • Demonstrates effective nursing skills (vitals, blood sugars, administering treatments and medications) in completing tasks accurately and efficiently.
  • Follows protocols and physician orders.
  • Demonstrates understanding of diverse patient care populations and ability to modify educational approach to ensure patient/family comprehension.
  • Evaluates for patient comprehension and documents response.
  • Demonstrates the ability to build trusting collaborative relationships with peers and the multi-disciplinary team to support patient care needs.
  • Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements.
  • Demonstrates task prioritization appropriately to facilitate quality patient care and ensure patient care needs are met.
  • Able to recognize and respond to emergency situations.
  • Evaluates own needs for support and uses identified systems/resources to obtain assistance.
