About The Position

Licensed Practical Nurse (LPN) I & II Enriched Housing at The Terrace at Glen Eddy. This is a part-time evening shift position, working every other Saturday and Sunday from 3p-11p. The Terrace at Glen Eddy is a unique, small, 42-bed assisted living facility located in Niskayuna, seeking an LPN to join their long-standing team. The environment is described as having a quality care focus without the heavy workload of a Skilled Nursing Home, emphasizing teamwork and preserving resident quality of life. The role involves medication administration and ADL assistance in an assisted living setting. The facility is conveniently located near Schenectady and Latham.

Requirements

  • Must be a Licensed Practical Nurse (LPN) in the State of New York.
  • Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).
  • 1+ years of medication passing (required).
  • 1+ years experience in Independent Living/Skilled Nursing Facility.
  • Must be able to stand, walk, and lift for long periods of time.
  • Must be able to read and write using medical terminology.
  • Working knowledge of computers including electronic medication administration records, processing of orders, checking email, and performing required in-servicing, along with use of other required applications.
  • Have the visual and auditory acuity adequate to perform observations and patient care.
  • Maintain composure and positivity in a high stress environment.
  • Promote physical, cognitive, and psychosocial well-being of each resident.
  • Work with residents in a highly personal manner during stages of acute and chronic illness.

Responsibilities

  • Provide medications, treatments, and staff oversight under the direction of the RN.
  • Bring trust, comfort, and empathy to all resident interactions.
  • Deliver compassionate, person-centered care while fostering a sense of dignity and independence for each resident.
  • Conduct daily medication pass with accuracy and care in accordance with physician orders and facility guidelines.
  • Provide leadership for the care team during your shift, including guidance and support.
  • Work closely with resident assistants, other nurses, and management in a cooperative, respectful environment.
  • Consult with the RN on any changes in a care plan or schedule.
  • Communicate with patients and their families about their care plan.
  • Observe, measure, record, and report clinical data relating to each resident's health status within licensure requirements.
  • Attend and actively participate in in-servicing, orienting, and educational activities.
