About The Position

Join us as an LPN and help champion compassion, dignity, and person‑centered support. Your care will directly enrich the lives of individuals with disabilities and empower them to thrive. This role involves providing nursing care in community and residential settings, training and supporting direct care staff, maintaining accurate medical records, and ensuring regulatory compliance. The LPN will also assist with staff training, monitor medication administration, participate in on-call rotations, and communicate with healthcare providers. Direct care responsibilities include assisting individuals with ADLs, community activities, household tasks, and medications.

Requirements

  • Graduate of an accredited school of nursing; holds a current Minnesota Nursing License.
  • Must have a valid Driver’s License and pass a driving record check conducted by our insurance carrier unless otherwise noted.
  • Excellent verbal and written communication skills, including the ability to effectively communicate with individuals and coworkers.
  • Must be self-motivated and exhibit superior interpersonal communication skills.
  • Display a positive attitude towards the job.
  • Must have a basic knowledge of technology programs. IE: Microsoft Office products. Be able to use and/or be willing to learn technology for daily charting.
  • Be a team player with the ability to work independently and carry out job duties as assigned.
  • Display exceptional documentation skills, and knowledge of systems, routines, and programming.
  • Must pass the MN Department of Human Services Background Study.

Responsibilities

  • Provide compassionate nursing care in community and residential settings.
  • Deliver care according to LPN licensure, policies, and individualized service plans.
  • Train, coach, and support direct care staff to ensure safe, high‑quality care.
  • Maintain accurate medical records and ensure regulatory compliance.
  • Help staff recognize illness symptoms and provide appropriate in‑home care.
  • Complete required health documentation and incident reporting.
  • Assist with onboarding and ongoing clinical training for staff.
  • Monitor medication administration, track errors, and support corrective actions.
  • Participate in scheduled on‑call rotation.
  • Communicate with physicians, pharmacies, and other healthcare providers.
  • Provide hands-on training in medication administration, CPR, and health‑related topics.
  • When providing direct care, assist individuals with ADLs, community activities, household tasks, and medications.
  • Maintain confidentiality and follow all HIPAA, safety, and infection control protocols.
  • Maintain active LPN licensure, CPR certification, and ongoing professional development.
  • Perform additional administrative or program support tasks as needed.
