Requirements

  • Active Minnesota Licensed Practical Nurse (LPN) license.
  • Ability to work independently while providing compassionate, patient-centered care.
  • Reliable vehicle, valid driver's license, and current auto insurance.

Nice To Haves

  • Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
  • EMR experience is a plus, but we'll help you learn our systems.

Responsibilities

  • Provide skilled nursing visits in patients' homes within approximately a 35-mile radius of your home.
  • Perform wound care, medication management, and other skilled nursing interventions.
  • Educate patients and family members on treatments, medications, and disease management.
  • Collaborate closely with RN Case Managers, physicians, therapists, and the interdisciplinary care team.
  • Document visits accurately and ensure continuity of care.
