As an LPN Visit Nurse, you'll care for one patient at a time in the comfort of their home—building meaningful relationships while practicing the clinical skills you worked hard to develop. And with patient visits scheduled within approximately a 35-mile radius of your home, you can provide care throughout your local area without being responsible for covering the entire Twin Cities. Requirements
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Ability to work independently while providing compassionate, patient-centered care.
- Reliable vehicle, valid driver's license, and current auto insurance.
Nice To Haves
- Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
- EMR experience is a plus, but we'll help you learn our systems.
Responsibilities
- Provide skilled nursing visits in patients' homes within approximately a 35-mile radius of your home.
- Perform wound care, medication management, and other skilled nursing interventions.
- Educate patients and family members on treatments, medications, and disease management.
- Collaborate closely with RN Case Managers, physicians, therapists, and the interdisciplinary care team.
- Document visits accurately and ensure continuity of care.
