About The Position

WorkFit Medical Staffing is seeking five (5) dedicated evening Licensed Practical Nurses (LPNs) with Long-Term Care experience to provide exceptional care to residents in a long-term care facility in the Southern Tier. As an LTC LPN, you will provide direct nursing care and support to elderly and disabled residents under the supervision of a Registered Nurse (RN) or physician. You will play a vital role in promoting resident health, comfort, and quality of life while collaborating with interdisciplinary healthcare teams.

Requirements

  • Graduate of an accredited Practical Nursing program.
  • Current, active New York State Licensed Practical Nurse (LPN) license.
  • Current BLS or CPR certification.
  • Minimum of one (1) year of Long-Term Care preferred experience.
  • Strong clinical, communication, and documentation skills.
  • Ability to work effectively in a fast-paced healthcare environment.

Responsibilities

  • Administer prescribed medications, treatments, and injections in accordance with physician orders and facility policies.
  • Accurately document all medications and treatments provided.
  • Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration.
  • Observe residents for changes in condition and promptly communicate concerns to the supervising RN or physician.
  • Assist residents with activities of daily living (ADLs), including bathing, dressing, eating, mobility, and personal hygiene.
  • Promote resident comfort, dignity, and independence whenever possible.
  • Provide basic wound care, including dressing changes and monitoring healing progress.
  • Report concerns or changes in wound status to the nursing team.
  • Work closely with Certified Nursing Assistants (CNAs) and other healthcare professionals to ensure quality resident care.
  • Assist in directing and supporting CNA staff to maintain high standards of care.
  • Maintain accurate and timely resident records using electronic medical record (EMR) systems.
  • Document assessments, interventions, observations, and resident responses to care.
  • In accordance with the New York State Nurse Practice Act: LPNs may perform a variety of nursing tasks and treatments within their scope of practice.
  • All nursing services must be provided under the direction of a Registered Nurse or licensed physician.
  • LPNs may not independently develop nursing care plans or make medical diagnoses.
