About The Position

Hartford HealthCare at Home, the largest provider of homecare services in Connecticut, is seeking a Licensed Practical Nurse (LPN) to join their team. This role is part of a person-centered care model, providing skilled nursing care to clients in home and community-based settings under the direction of an RN. The LPN will ensure the delivery of outcome-based, high-quality, cost-effective care and collaborate with team members to achieve exceptional outcomes. The position involves providing intermittent skilled nursing services, supporting patients and families through teaching, recording daily progress in EMR, reporting to various healthcare professionals, and consulting with physicians. The LPN will also assist in evaluating paraprofessional staff and participate in agency committees.

Requirements

  • High School diploma or GED
  • Successful completion of a CT state approved LPN certificate program
  • Computer skills including but not limited to Outlook, Microsoft Word, and Excel
  • Current and valid LPN license
  • Valid CT driver’s license & a good driving history

Responsibilities

  • Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
  • Provide support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.
  • Record daily progress in EMR.
  • Report to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
  • Communicate/conference with primary nurse and other disciplines.
  • Consult with physician regarding client needs and plan of care.
  • Assist in evaluation of care given by paraprofessional staff.
  • Participate on agency committees and special projects.
