About The Position

Putnam Nursing & Rehabilitation Center, a 160-bed skilled nursing and rehabilitation facility in Putnam County, New York, is seeking a Licensed Practical Nurse (LPN). The facility is newly renovated and under new nursing leadership, dedicated to maintaining excellence and integrity. They focus on providing high-quality care with respect and compassion through an interdisciplinary approach, aiming for personalized goals to achieve maximum potential and quality of life for residents. This position offers a significant retention bonus, paid in weekly installments, which breaks down to over $3.00/hr more in the first 26 weeks. Excellent 1199 Union benefits are also provided.

Requirements

  • Graduate of an approved school of Nursing.
  • Licensed and currently registered in good standing in New York as an LPN.
  • Dedicated to providing extraordinary, clinically sound, and compassionate care.
  • Must possess a spirit of cooperation, caring, and enthusiasm to create an atmosphere conducive to rehabilitation and optimal resident health.
  • Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
  • Have excellent communication, customer service, and organization skills.
  • Work well under pressure and willing to work harmoniously with all staff.

Responsibilities

  • Provide daily support to unit leadership and respond appropriately and timely to resident/family needs within the LPN scope of practice.
  • Use nursing process, judgment, and skills to set priorities necessary to deliver quality direct resident care and make decisions promoting safety and quality direct care to residents.
  • Perform and administer standard nursing treatments and/or medications and observe residents for desired or unwanted effects.
  • Communicate and collaborate with appropriate members of the care team to revise/update the Nursing Care Plan or any pertinent findings, including lab/test results, incidents, and/or concerns.
  • Monitor and document vitals, treatments administered, and residents’ responses.
  • Prepare, organize, oversee, monitor, and evaluate the delivery of care of resident CNA assignments and activities, considering the category of needs and competency of personnel.
