About The Position
StoryPoint Crystal Lake is seeking a Licensed Practical Nurse (LPN) to join their team. This role involves leading, managing, and training a team of caregivers, evaluating residents, and reporting changes in condition to physicians. The LPN will utilize Electronic Health Records (EHR) and Electronic Medication Administration Records (EMAR), communicate effectively with the team, residents, and families, and build strong relationships. Requirements
- Licensed Practical Nurse (LPN) in the state.
- Prior supervisory experience required.
- Punctuality and professionalism are important factors.

Nice To Haves
- Assisted Living/Memory Care experience preferred.
