Requirements

At least 1 year experience with either LTAC or Surgical unit
Current Louisiana license
Must be able to work flexible schedule including 12 hour shifts
Have dependable transportation, drivers license and car insurance

Responsibilities

Provides nursing and supportive care in accordance with Interdisciplinary Plan of Care, under the direction of the RN.
Communicates with the attending physicians, other hospice team members and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
Document all services provided and submit on appropriate forms.
Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
Report changes in patient condition and needs to RN
