About The Position

The LPN provides quality nursing care and services to residents in a respectful and dignified manner, ensuring the delivery of care and services. The LPN coordinates physical, social, and emotional aspects of assisted living care and services in accordance with the physician’s orders and the resident’s choices. This role involves executing the medication management plan, continuously assessing resident needs, implementing measures for resident wellness and safety, and working collaboratively within a team to meet resident needs. The LPN is also responsible for responding to emergencies, reporting incidents, participating in community activities, assisting with meal delivery, maintaining high standards of record-keeping, identifying resident risk factors, providing thorough shift reports, and motivating CNAs/RMAs. The position requires adherence to all policies and procedures, practicing proper body mechanics, and maintaining attendance standards.

Requirements

  • Current LPN license to practice nursing in the state in which the community is located, holding a license in good standing with the Board of Nursing.
  • Experience working with the senior population, preferably in assisted living or LTC.
  • Must be a team player who works well with others and has experience supervising staff.
  • Must possess critical thinking skills.
  • Ability to effectively communicate verbally and in writing.
  • Demonstrated ability to manage time efficiently with strong organizational skills.
  • Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
  • Must be willing to work an 8-16 hour flexible schedule to include weekends and holidays.
  • Must attend all required trainings, CEU’s and staff meetings.
  • Must be certified or willing to take and pass the certification class required by the state in which employed.

Responsibilities

  • Provides quality nursing care and services to residents in a respectful and dignified manner.
  • Coordinates physical, social, and emotional aspects of assisted living care and services.
  • Executes the medication management plan, including periodic review of documentation and communication with physicians.
  • Continuously assesses resident needs and updates appropriate individuals regarding care, services, and changes in condition.
  • Implements measures to ensure resident wellness and safety and documents these measures.
  • Works in a team to meet resident needs.
  • Responds to emergencies calmly and competently.
  • Reports incidents and changes in resident condition to the Healthcare Director with appropriate documentation.
  • Participates actively in all activities, including community events.
  • Assists with meal delivery in the dining room, monitoring resident acceptance of diets and ensuring adequate nutrition.
  • Maintains high standards with regard to record keeping, resident and staff documentation.
  • Establishes and maintains a positive working relationship with all departments.
  • Identifies risk factors for residents and initiates interventions with appropriate documentation.
  • Provides a thorough report of resident status to the on-coming nursing team at each change of shift and in the Daily Log.
  • Provides motivation and direction to CNA/RMA’s.
  • Attends required trainings, CEU’s and staff meetings.
  • Adheres to and implements all policies and procedures.
  • Practices proper body mechanics and safe resident transferring techniques.
  • Adheres to attendance policies; reports to work on time and prepared to begin duties.
  • Completes all other assigned duties.
