Requirements

Active Licensed Practical Nurse (LPN) license
Experience in a rehab setting required
Successful completion of a post-offer, pre-employment criminal background check and physical/drug screen

Experience with short-term and short-stay patients

Responsibilities

Provide nursing care to patients in a Transitional Care Rehab Unit.
Manage short-term and short-stay patient needs.
Administer medications and treatments as prescribed.
Monitor patient conditions and report changes to physicians.
Collaborate with the healthcare team to develop and implement patient care plans.
Educate patients and their families on health conditions and treatment plans.
