Licensed Practical Nurse (LPN) Unity Living Center (Full-Time, Nights)
About The Position
The Licensed Practical Nurse provides professional care to long-term and post-acute care residents at Unity Living Center, a 120-bed skilled nursing facility. The role includes directing and monitoring CNA-provided care and ensuring accurate documentation. The facility cares for individuals with complex medical needs, dementia, and behavioral challenges, and offers opportunities to develop clinical expertise in areas such as short-term rehabilitation, IV therapy, peritoneal dialysis, hemodialysis, ventilator care, pain management, and chronic disease management. This is a full-time, 40-hour-per-week night position, scheduled from 11:00 p.m. to 7:00 a.m., at Unity Living Center in Rochester, New York.
Requirements
- Graduate from an accredited School of Practical Nursing.
- Current LPN licensure in New York State.
- Basic Life Support (BLS) certification from the American Heart Association.

Responsibilities
- Administer medications to the assigned team accurately and on time, with required documentation.
- Perform treatments according to physician orders and facility protocols, and document progress.
- Maintain complete and pertinent resident-care documentation in the computerized medical record and initiate appropriate care pathway tools.
- Receive and provide relevant written and verbal reports to appropriate staff in accordance with departmental protocols.
- Monitor residents and report relevant changes in condition to the charge nurse, supervisor, and/or physicians in a timely manner.
- Complete pre-reviews of medication administration records according to facility protocols.
- Complete required MDS documentation and related observations in a timely manner.
- Support the CNA team by assisting with residents’ activities of daily living.
