About The Position

This position provides dedicated one-to-one licensed nursing support for an elementary-age student with significant neurological, physical, respiratory, feeding, and communication needs. The LPN will work within their scope of practice and under the direction of a certified school RN. The student is dependent on adults for nutrition via a gastrostomy tube, safe positioning, mobility, monitoring, personal care, and communication of discomfort or changes in condition. Key daily needs include frequent G-tube feedings, seizure monitoring, possible oral suctioning, respiratory observation, positioning, personal care coordination, and emergency readiness.

Requirements

Licensed Practical Nurse (LPN) license.
Ability to work within scope and under the direction of a certified school RN.
Experience with G-tube feedings.
Experience with seizure monitoring and emergency seizure medication administration.
Experience with respiratory observation and secretion management.
Ability to perform oral suctioning.
Proficiency in patient positioning and mobility assistance.
Skills in personal care coordination.
Ability to maintain continuous readiness for emergencies.
Familiarity with documenting medical procedures and observations.
Strong communication skills for reporting to RN, family, and providers.
Ability to follow prescriber's orders and care plans accurately.
Knowledge of emergency action plans and EMS activation procedures.

Nice To Haves

Experience working with students with complex medical needs.
Familiarity with quadriplegic cerebral palsy and global developmental delay.
Experience with students with limited speech and communication.
Knowledge of pulmonary hypertension, obstructive sleep apnea, and sleep-related hypoventilation.
Experience with cardiac, renal, and metabolic conditions.

Responsibilities

Perform frequent G-tube feedings as ordered.
Monitor for and respond to seizure activity, including administering emergency rescue medication as prescribed.
Perform oral suctioning as needed and ordered.
Continuously observe and monitor the student's respiratory status, including breathing effort and secretion management.
Ensure proper positioning, mobility, and personal care coordination with classroom staff.
Conduct arrival and baseline reviews, assessing general appearance, alertness, respiratory effort, secretion management, positioning, G-tube site, feeding tolerance, seizure activity, and availability of medications and supplies.
Document all feedings, water intake, tolerance, vomiting, suctioning, seizures, medication administration, assessments, interventions, and communications.
Communicate significant information to the certified school RN, family, provider, or EMS according to the care plan.
Prepare feeding and suction equipment, check rescue medication expiration dates, and verify backup/EMS procedures before the student arrives.
Complete documentation, communicate significant information, secure medications and supplies, and transfer responsibility according to the transportation and parent handoff plan before dismissal.
