About The Position

This position provides dedicated one-to-one licensed nursing support for an elementary-age student with significant neurological, physical, respiratory, feeding, and communication needs. The student is dependent on adults for nutrition via a gastrostomy tube (G-tube), safe positioning, mobility, monitoring, personal care, and communication of discomfort or changes in condition. The student has a complex seizure disorder with prescribed emergency rescue medication, quadriplegic cerebral palsy, global developmental delay, limited speech, G-tube dependence, pulmonary hypertension, obstructive sleep apnea, sleep-related hypoventilation, and other medical complexities. The LPN will work within their scope and under the direction of the certified school RN.

Requirements

Licensed Practical Nurse (LPN) license.
Ability to work within scope and under the direction of a certified school RN.
Proficiency in G-tube feedings.
Experience with seizure monitoring and emergency protocols.
Ability to perform oral suctioning.
Skills in respiratory observation.
Competence in positioning and mobility assistance.
Ability to coordinate personal care.
Readiness for emergency situations.
Familiarity with managing complex medical conditions including neurological, physical, respiratory, and feeding needs.
Ability to document accurately and communicate effectively with healthcare team, family, and school staff.
Understanding of G-tube dependence and care.
Knowledge of pulmonary hypertension, obstructive sleep apnea, and sleep-related hypoventilation management.
Ability to administer intranasal diazepam as prescribed.
Capability to assess and manage vomiting and potential aspiration risks.
Skills in patient and family education and support.

Responsibilities

Perform frequent G-tube feedings.
Monitor for seizures and administer emergency rescue medication as needed.
Perform oral suctioning if necessary.
Conduct respiratory observations.
Assist with positioning and mobility.
Coordinate personal care.
Maintain emergency readiness.
Assess general appearance, alertness, respiratory effort, secretion management, positioning, G-tube site, feeding tolerance history, seizure activity, and availability of medications, supplies, and emergency equipment.
Administer G-tube bolus feedings as ordered.
Perform pre-feeding observations, including positioning and G-tube site inspection.
Monitor during and after feeding, documenting response and managing intolerance.
Monitor for seizures, time events, protect from injury, support airway, observe breathing and color, provide post-seizure care, and document/report events.
Administer emergency seizure medication according to orders and Emergency Action Plan, activating EMS when administered.
Respond promptly to vomiting or choking risk, protect airway, assess respiratory status, and perform oral suctioning as ordered.
Monitor for increased work of breathing, ineffective secretion clearance, color change, reduced responsiveness, choking, aspiration signs, or other departures from baseline.
Coordinate safe positioning, transfers, hygiene, toileting, and equipment needs with classroom staff.
Document feedings, water, tolerance, vomiting, suctioning, seizures, medication, assessments, interventions, and communications.
Notify the certified school RN, family, provider, or EMS according to the care plan.
Review daily health information, confirm orders and plans, check rescue medication, prepare equipment, and verify backup/EMS procedures before student arrival.
Complete and document a baseline observation upon student arrival, confirming overnight status.
Remain available for feedings and post-feeding observation while continuously monitoring seizure, respiratory, positioning, and communication needs throughout the school day.
Accompany or remain immediately available during instruction and activities as required by the RN-developed plan.
Protect tubing and equipment and coordinate access without unnecessarily limiting participation.
Complete documentation, communicate significant information, secure medications and supplies, and transfer responsibility according to the transportation and parent handoff plan before dismissal.
