About The Position

Eventide on Eighth is hiring for a Full-Time LPN to work the PM (8 hours) shift (2:30p-11p), with every third weekend and holiday rotation. This role involves assessing residents' needs, providing direct nursing care, administering medications and treatments, and ensuring accurate documentation. The LPN will also play a role in updating care plans, coordinating with healthcare professionals, and supporting resident and family education. Additionally, the position requires ensuring adequate staffing, monitoring subordinate staff activities, and maintaining up-to-date nursing skills.

Requirements

  • Must have an active MN LPN license or be able to obtain upon hire.

  • Experience in Long Term Care preferred, but not required.

Responsibilities

  • Assess residents’ needs and health status through physical assessments, observations, and interviews.
  • Provide individual attention to encourage residents' highest physical, mental, and psychosocial well-being.
  • Observe and follow residents’ plans of care, updating and making appropriate changes when necessary.
  • Administer medications and treatments and monitor for effectiveness.
  • Provide direct nursing care to residents as needed, according to established policies and procedures.
  • Comply with rules and regulations set forth by the Minnesota or North Dakota Department of Health and the Center for Medicare/Medicaid services.
  • Provide accurate documentation in the residents’ electronic medical records.
  • Assist in the orientation/training of new team members.
  • Report resident needs, problems, and observations to the Charge Nurse and/or attending physician.
  • Ensure adequate staffing levels for a scheduled shift and correct inadequate staffing appropriately.
  • Monitor the activities and functions of subordinate staff, potentially participating in the disciplinary process.
  • Ensure work responsibilities of subordinate staff are completed thoroughly and accurately.
  • Provide information, education, and support for assigned residents and their families.
  • Communicate the needs of residents to resident care staff in the unit.
  • Interact with physicians and other health care facilities as needed.
  • Continually update nursing skills and knowledge by attending in-services, continuing education programs, and staff meetings.
  • Coordinate and participate in resident care conferences and communicate information effectively to families.
  • Assume responsibility for promptly reporting any incidents or evidence of abuse, neglect, or violation of residents’ rights per policy.
  • Manage electronic timecards daily.
  • Carry out or assist with other tasks as occasionally required.
