About The Position

TheKey Home Health Services is seeking an adaptable Licensed Practical Nurse to provide home health care to clients across Fairfield County, Connecticut. Primary assignments involve standard home visits, individualized care plans, routine wellness checks, and direct clinical care. The nurse accepts assignments on an as-needed basis, with priority given to clinicians available for last-minute requests or temporary coverage gaps. The position may also transition into extended-shift or block-coverage opportunities as regional care volume expands.

This per-diem, on-call, part-time position offers compensation starting at $42.00 per hour, varying by client and shift. TheKey describes the role as providing clinical autonomy in the field, 24/7 team support, and meaningful, personalized home care on a flexible schedule.

Requirements

• Active, unencumbered Licensed Practical Nurse (LPN) license in Connecticut.
• Minimum 1–2 years of clinical nursing experience.
• Current CPR/BLS certification.
• Flexible availability and willingness to work per-diem shifts, on-call visits, and last-minute assignments.
• Valid Connecticut driver’s license, reliable vehicle, and active auto insurance.
• Strong clinical assessment skills within LPN scope of practice.
• Effective communication skills.
• Proficiency with Electronic Health Records (EHR) for daily charting.

• Background in home health, geriatrics, or care management.

Responsibilities

• Provide skilled nursing care, routine wellness visits, medication administration, wound care, and health monitoring in accordance with physician orders and established plans of care.
• Observe, record, and promptly report changes in patient conditions, symptoms, or behavior to the supervising RN or attending physician.
• Accept per-diem visit assignments as needed and respond flexibly to coverage needs across Fairfield County.
• Build rapport with clients, families, and multidisciplinary care teams while providing health education and empathetic support.
• Complete and submit accurate, timely clinical notes according to agency policy, state home health regulations, and HIPAA standards.
• Transition into extended-shift or block-coverage opportunities as regional care volume expands.
